Upon admission to Rouses Group Home ICF/IDD services, assessments are completed by various health care disciplines to identify the needs and services that best meets the individual’s person centered plan. These individualized programs are done with the collaboration of the individual, their family or legally responsible person and their interdisciplinary team comprised of Speech Therapists, Psychologists, Nurses, Physical Therapists, Habilitation Specialists, Social Workers, Dieticians, Physical and Occupational Therapists, Recreational Therapists, Qualified Developmental Disability Professionals (QP), and other disciplines as needed.
